If you already have an NHS Log In it is easier to register, if not then you will need to provide your personal details to register. 
Patients with the NHS Log In will only need to add their Sex and Postcode.

Step 1.
Go to Oakley Health website: https://www.oakleyhealth.org/

Step 2.
Click on the RED bar at the top to connect to and register with the Anima system, this will take you to: https://patients.animahealth.com/signUp

• If you have an NHS Log In then click the ‘I already have an account’, this will take you to the pages to log in using your NHS Log In.
  
• If you do not have an NHS Log In, provide the requested information and then press Create account.
  The next step is to follow the instructions and provide all your personal information.

Step 3
When you have registered you can return to Oakley Health
website [ https://www.oakleyhealth.org/ ]  scroll down their Homepage and click on any of the action icons which will then return you to the Log In screen.
